Last Date of Submitting Filled up Application form is March 31, 2015. You must submit your filled up application form near the FSIB branch of your permanent address. Otherwise your application will be cancelled.

• One (01) copy PP size attested photo by your Institutional Head.
• Father/Mother/Guardian’s yearly income certificate from two source. One is from your present institutional Head and the another is from your local Ward Commissioner or Local Chairman. If a guardian is employed then he or she must provide the yearly income certificate given by their upper legal authority. Hope you understood.
